“Personal Information” refers to any data from or about an individual that either directly identifies a person or is reasonably likely to make a person identifiable when combined with other information from or about that person from any source. We do not consider information that can no longer be used to identify a specific person – for instance, de-identified, pseudonymized, anonymized, or aggregate information about you or our other consumers – as Personal Information. Moreover, where we de-identify your Personal Information, we pledge to maintain that data in de-identified format.

A. Information You Supply to Us.

  • Personal Information you may provide us through the Service includes:

  • Contact data - such as your name, email address, billing and mailing addresses, and telephone number

Your responses are not shared with third-party advertising or marketing programs; however, certain marketing and advertising services are automatically notified when a survey is completed for business analytics, advertising, and marketing purposes.

These third parties may employ cookies, web beacons, device identifiers, and other technologies to gather information about your use of our Services and other websites and mobile apps – this information may include your IP address, web browser, mobile network information, pages viewed, actions completed on the website and in our mobile app, time spent on pages or in mobile apps, links clicked, and conversion information. This data allows us to determine whether you have previously visited the Service, remember preferences you have set, statistically monitor the number of people using our Service and for what purposes, and enables the delivery of advertising targeted to your interests. When you purchase a service from the Anywhere Clinic website, web beacons, pixels, or API from our service providers’, advertising providers’, and marketing providers’ analytics tools may capture and transmit the conversion data to help assess the efficacy of advertising campaigns and inform future advertising.
